Emerging Technologies in Diabetes and Cardiometabolic Health

Research Works
A revolution in the treatment of diabetes lies in the use of artificial intelligence to better tailor insulin delivery to the patient’s lifestyle via a closed-loop (CL) system (“artificial pancreas”). In both randomized and real-world data, this technology aims to increase the time spent in the euglycemic target (TIR 70-180 mg/dl) by about 10 percentage points (pp) mainly by limiting hyperglycemia, and to lower exposure to hypoglycemia (<70 mg/dl) by about 1.5-2 pp, with a concomitant decrease in HbA1c of about 0.3-0.4 pp. More efficient glucose homeostasis could delay or decrease micro- and macro-vascular complications via better balance of inflammatory and oxidative status. However, it should be noted that the knowledge about the predictors of good metabolic response to these closed-loop hybrid systems are currently limited in routine practice, although a recent study reported in a pediatric population that initial TIR 70-180 mg/dl, initial HbA1c, and the patient’s investment in daily insulin bolus management predicted improvement on euglycemic status over 12 to 16 weeks. The objective of our research based on data from real-life studies (participative research) is i) to find the different predictors involved in an optimal glycemic regulation at the individual level (including dietary profile) as well as ii) to investigate the benefits of CL system on lipid metabolism using fast-GC methodology (fatty acid status) and NMR (metabolic signature), in the purpose of a personalized medicine.
